Insurance

We ask that parents be aware of their insurance benefits, deductibles, and limitations prior to visiting our office.  There are hundreds of insurance plans, and we do not have the ability to know your exact benefits.  We also ask that you have your insurance card/employer information with you when you come to your visit, and provide any information updates as they change, so that we may accurately submit claims to your insurance.

We are an Out of Network” provider with all insurance plans.  Some insurance plans will require that you see a provider who is In Network” only to be able to use your benefits.

You would want to check with your insurance company to see if your plan has Out of Network benefits.  Please note that if they tell you they will pay 100% of checkups, 80% of basic treatment, etc., that this is based on their ‘allowed’ amounts under the plan, and not necessarily our charges.  Some will pay 100% of our charges and some will pay at a lesser percentage.

Many plans that have Out of Network benefits will pay our office directly, and then we bill the family for any balance not paid by insurance and ask that the balances be paid within thirty (30) days of our billing date.  If your insurance plan will not pay our office directly, then we require payment at the visit.  We will submit a claim on your behalf to your insurance company to reimburse you.

If you are Self-pay/No insurance – payment is due at the time of visit.  Our office accepts Visa, Mastercard, Discover, HSA/Benefits Card, cash or check payment.
